ICICI Lombard Health Insurance Complaints

ICICI Lombard Health Insurance (popularly known as Mediclaim)

Coverage: ICICI Lombard Health Insurance do not cover full Hospital Bill. Whenever I was hospitalized, Whenever I used ICICI Lombard Health Insurance, people who are working in hospital billing department told me to pay some bill (some amount) in cash. Why ICICI Lombard Health Insurance can not cover all Hospital Bill? If they (ICICI Lombard) can cover all hospital bill, I am willing to pay more premiums.
